Use this form to request a background check for anyone that will be on campus for more than three days. This applies to students, student applicants, resident students, visiting students, employees, prospective employees, temporary employees, visitors, vendors and contractors. If you need to submit a request for multiple people, they will need to be listed on this spreadsheet template found in the Attachments section of this service request page. The file must be password protected and the password provided separately to backgroundchecks@uthscsa.edu.
