What is Microsoft Forms?
Microsoft Forms is a simple form or quiz creating tool that keeps records and data entered within itself to be displayed graphically or exported as an excel document. It can be accessed on any web browser and on mobile devices.
Accessing Forms:
- Go to MyUTHealth - https://uthealthsa.sharepoint.com/Pages/Home.aspx
- Click on the waffle icon on the top left
- Click Forms

When to use Forms:
- For quick, easy to use forms without a need for a data source
- No need for strict permissions

- Displays forms and quizzes created by groups
Form
Clicking on New Form will create a new form for editing. You can name your form, enter a description for your form, and add an image if desired. The image will be automatically cropped to a square image and placed to the left of the title.
Clicking on Add question will provide you with various kinds of response types. Each response type has the option to make it required, add a subtitle, and add an image, which will be displayed to the right of the question or under the question depending on what platform the form is being displayed on. Questions can be copied, deleted, and moved up or down to change the order of the questions in the form. The following are the response types available:
- Choice
- Text
- Rating
- Date
- Ranking
- Likert
- Net Promotor Score
Quiz
Clicking on New Quiz will create a new quiz for editing. Quizzes have the same response types as forms. Additionally, it has the option of identifying the correct answer or answers and a section for setting the number of points awarded when choosing the correct answer.
