Department ID: Department that the user being assigned the equipment is in
Business Purpose: Work From Home
Off Campus Location: Enter the users physical home address where the equipment will be located
Date of Removal: Date of Removal
Anticipated Return Date: Anticipated Return Date
Browse to location where Excel spreadsheet is saved to attach to the service request.

Employee ID: Badge number
Employee Removing Equipment: Employees first and last name
Property Custodian: Same as Requester
Department Designee/Manager/Director/Chair: Employees Manager or Director
